Experimental Studies on the Effect of Endotracheal Hydrocortisone Instillation in Acid Aspiration Pneumonitis


Abstract
The study was carried out to determine the effect of hydrocortisone by means of 3ntratracheal instillation upon chemically produced pneumonitis, using 12 normal rabbits, weighing 1.35 kg to 1.85 kg.
The agents used to produce chemical pneumonitis were HCl, pH 1.5 and pH 2.5 solutions. The hydrocortisone used for the experiment was solu-cortef 100mg in 2ml solution. All animals were sacrificed and examined 30 minutes after the procedure.
The results were as follows:
1. In the group of chemical pneumonitis induced by HCl, pH 1.5 solution, severe hyperemia and edema was noted throughout microscopic field. Marked inflammatory reaction is also noted in the alveolar wall, numerous erythrocytes were extravasated in the alveolar lumen.
2. The inflammatory reaction in the lung was much less in the group of HCI, pH 2.5 solution.
3. In the group in which only hydrocortisone instilled, no pathologic finding was observed.
4. In the group of HCI. pH 1.5 induced pneumonitis pretreated with aydrocortisone, mild inflamm-
atory reaction and edema were noted.
5. In the group of animals which were treated with hydrocortisone by means of intratracheal instillation after the HCI, pH 1.5 induced pneumonitis, only mild hyperemia and edema in the right lobe were noted and alveoli appeared to be rather indistinguishable from those of normal.
